Steps

  1. Use a binder with tabbed sections and a clear cover page with the person's name.
  2. Add a contact sheet: family, doctors, pharmacy, neighbors and 911 with the home address.
  3. Add the current medication list and allergies.
  4. Add copies of insurance cards, the health care proxy and the power of attorney.
  5. Add a daily routine page: meals, mobility, what helps and what upsets.
  6. Keep a log of changes, falls, appointments and questions for the doctor.
  7. Keep it in one agreed place, photograph each page, and date every update.
